I hereby certify that all the information completed on this official application regarding named contestant are correct and accurate; and that said contestant will participate in appropriate age group. I understand there is a $20 registration fee to process this application. If applicant is accepted as a finalist, I am aware there is a pageant fee for $300 to participate. It is understood that all fees are non-refundable without exception. In agreement of participating as a contestant in this pageant, we the parents and/or legal guardians of the contestant named on this application, do hereby release Miss Lucky Star Pageant, it's director, staff, employees and sponsors from any and all claims and damages directly or indirectly resulting from named contestant's participation. We give permission to Miss Lucky Star Pageant to use the contestant's photographs, speeches, testimonials, videos, etc. for publicity purposes and future materials. It is agreed and understood that any scholarships and prizes provided are determined by the sponsors. Miss Lucky Star Pageant shall have no liability in regard to such prizes or scholarships. Neither the contestant nor any of her immediate family members are employed by Miss Lucky Star Pageant.
